Our Approach to Collaborative Care

You are a great generalist and we are here to be your mental health specialist (we’re weird enough to love it).

We work closely with referring providers to ensure continuity and clarity.

Our goal is to integrate, not interfere. When you refer a patient, you stay in the loop with timely updates on med changes and relevant clinical notes. We’re available for curbside consults with a dedicated contact line, care coordination, and questions—because mental health is a team sport, and we play well with others. Here's what collaboration looks like:

Transparent communication (no guessing games)

Respect for your treatment plans

Psychiatric support that enhances, not complicates, your workflow

Care that aligns with best practices and common sense

Let's be Partners in Care

We know what it’s like to juggle  20+ patients, and endless admin tasks. Add in the unique challenges of mental health disorders, and it can make for a long day. We can’t fix the admin side (we wish!) but we’re pretty great with mental health.  Let us be your go-to mental health partner—so your patients get the specialized care they need, and you get to focus on everything else you’re great at.

Have questions or want to talk through a complex case before referring? Give us a call. We’re real humans (and yes, we call back).

Dr. Kati Wijdenes, DNP, PMHNP-BC
Psychiatric Nurse Practitioner. Air Force Veteran. Your mental health co-pilot.

I specialize in working with real people facing real mental health struggles—and I do it with empathy, clinical precision, and just enough humor to keep things grounded.

With over a decade of experience in psychiatric care and medication management, I enjoy working closely with referring providers to ensure seamless, high-quality treatment that respects the therapeutic relationship you’ve already built.

I believe in clear communication, compassionate care, and collaboration that doesn’t create more work for you. Let’s team up.

When is it time to refer a patient?

We know you're juggling a lot — and not every patient needs a referral to a specialist. But here are some signs that it's time to bring us in:

The treatment plan is stalling

If your patient isn't responding to first-line interventions, it might be time to co-manage with a psychiatric specialist. We're here to help keep them moving forward.

Mental health is disrupting your primary care plan

When your patient’s mental health symptoms, like anxiety or depression, begin interfering with their physical care plan, we can step in to stabilize symptoms and support their overall progress.

The diagnosis feels outside your scope

We frequently assist with complex presentations like ADHD, PTSD, bipolar disorder, and personality disorders. If you’re hearing phrases from your patient like "I’ve always been this way" or "I just feel broken," let’s talk.

They need meds you can’t (or don’t want to) manage

If you're hesitant to prescribe certain psychiatric medications, we get it. We're comfortable managing psychotropics. We'll collaborate with you to avoid polypharmacy pitfalls.

The patient just needs more time

Mental health deserves more than a rushed 15-minute visit. We offer 60-minute intakes and regular follow-ups to dig deeper into what’s going on, so you can stay focused on the rest of their care.
